Self Care Information

NHS 111 Service

NHS Out of Hours Service provides medical care or advice when the surgery is closed. Fully trained advisers will assess your symptoms and direct you to the most appropriate local service. This can include an out-of-hours doctor, walk-in centre, urgent care centre, emergency dentist or late opening chemist. For life threatening emergencies, continue to call 999.

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Please note we are part of the NHS Sumamry Care Records Service, for further information see the Summary Care Record section on our website.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
